When purchasing a foreign used car, the radio inside is not tuned to work with the radio stations on this side of the world. This device does the necessary conversion without the expense of changing the radio. Changing the radio is also a hassle sometimes because the original is nicely built into the car's dash board and if changed, the replacement will look awkward.
